DELAYED ONSET MUSCLE SORENESS (DOMS)
Delayed Onset Muscle Soreness (DOMS) is the muscle pain and stiffness that develops after exercise, usually beginning 12–24 hours after activity, peaking around 24–72 hours, and gradually resolving over several days.
DOMS is especially common after:

Starting a new exercise program

Returning to exercise after a break

Increasing training intensity or volume
-
Performing exercises with a strong eccentric (muscle-lengthening) component
Causes of DOMS
DOMS is not caused by lactic acid buildup. Lactic acid is cleared from muscles within hours after exercise.
The current understanding is that DOMS results from:
Microscopic Muscle Damage
Exercise can create tiny disruptions in muscle fibers and surrounding connective tissue.
This is particularly common during:

Downhill running

Lowering weights slowly
-
Jumping activities
-
High-intensity resistance training
Inflammatory Response
The body responds to the microscopic damage by:

Sending immune cells to the area

Initiating tissue repair processes
This contributes to soreness and stiffness.
Eccentric Muscle Contractions
DOMS is most strongly associated with eccentric contractions, where a muscle lengthens while producing force.

Lowering a dumbbell during a bicep curl

Descending stairs

Running downhill

Lowering into a squat
Symptoms of DOMS
Muscle Soreness
The hallmark symptom is:

Tender, aching muscles

Pain when moving or using the affected muscles
Stiffness
You may notice:

Difficulty moving normally

Feeling "tight" after resting
Tenderness to Touch
Affected muscles may be:

Sensitive when pressed

Temporary Weakness
DOMS can cause:

Mild Swelling
Some people experience:

Slight swelling

A feeling of fullness in the muscle
Reduced Range of Motion
Because of soreness and stiffness:

DOMS vs. Injury
DOMS Usually Causes:

Significant swelling or bruising

Pain in joints, ligaments, or tendons

Symptoms that worsen rather than improve
When to Seek Medical Attention
DOMS is usually harmless, but seek medical care if you experience:

Severe swelling

Extreme weakness
-
Dark or cola-colored urine
-
Severe pain that is out of proportion to the workout
These symptoms can occasionally indicate a serious condition called Rhabdomyolysis.
What Helps DOMS
Evidence suggests the following may help reduce discomfort:

Light activity (walking, cycling)

Gentle stretching
-
Adequate sleep
-
Good hydration
-
Proper nutrition, especially sufficient protein
-
Sports massage or massage therapy
-
Gradual progression of training load
The good news is that as your body adapts to an exercise, the same workout usually causes much less soreness in the future. This is known as the repeated-bout effect, where muscles become more resistant to DOMS after they've been exposed to a particular type of exercise.
