IT BAND SYNDROME
IT band syndrome (Iliotibial Band Syndrome, or ITBS) is one of the most common causes of pain on the outside of the knee, especially in runners and cyclists.
The iliotibial (IT) band is a thick band of connective tissue that runs from the outside of the hip, down the outer thigh, to the outside of the knee. When the tissues around the IT band become irritated from repetitive movement, pain develops on the outer side of the knee.
Iliotibial Band Syndrome
Causes of IT Band Syndrome
Overuse
The most common cause is repetitive bending and straightening of the knee.
Common activities include:
-
Running
-
Cycling
-
Hiking
-
Rowing
-
Long-distance walking
Sudden Increase in Training
Examples include:
-
Increasing running mileage too quickly
-
Adding hill workouts
-
Increasing cycling volume
-
Returning to sport after a break
Weak Hip Muscles
Weakness in:
-
Gluteus medius
-
Gluteus maximus
can allow the knee to move inward excessively during activity, increasing stress on the IT band.
Tight Surrounding Muscles
Tightness in:
-
Tensor fasciae latae (TFL)
-
Gluteal muscles
-
Hip flexors
-
Quadriceps
may contribute to abnormal tension in the IT band region.
Running Mechanics
Factors that may increase risk include:
-
Excessive inward knee collapse
-
Overstriding
-
Poor running form
-
Running on sloped surfaces
Foot and Ankle Issues
Examples:
-
Excessive pronation
-
Limited ankle mobility
-
Poor footwear
Cycling Factors
For cyclists:
-
Saddle height errors
-
Cleat positioning issues
-
Bike fit problems
can contribute to IT band irritation.
Symptoms of IT Band Syndrome
Pain on the Outside of the Knee
The hallmark symptom is:
-
Sharp or aching pain on the outer side of the knee
This is usually the most noticeable symptom.
Pain During Exercise
Common pattern:
-
Little or no pain at the beginning
-
Pain develops after a certain distance or time
-
Symptoms worsen as activity continues
Pain While Running Downhill
Many runners notice:
-
Increased pain during downhill running
-
Increased pain when descending stairs
Tenderness
The outside of the knee may:
-
Feel tender when pressed
-
Be sensitive after exercise
Tightness Along the Outer Thigh
Some people experience:
-
Tightness from the hip to the knee
-
A feeling of pulling on the outer leg
Reduced Performance
Symptoms can make it difficult to:
-
Continue running
-
Cycle comfortably
-
Perform longer workouts
Typical Progression
Early Stage
-
Pain only during longer activities
-
Symptoms settle quickly after stopping
Moderate Stage
-
Pain appears sooner during exercise
-
Recovery takes longer
Advanced Stage
-
Pain during daily activities
-
Pain walking downstairs
-
Symptoms persist after exercise
Common Activities That Trigger Symptoms
People often report pain when:
-
Running
-
Running downhill
-
Walking downhill
-
Cycling
-
Climbing stairs
-
Descending stairs
IT Band Syndrome vs. Runner's Knee
IT Band Syndrome Runner's Knee
Pain on outside of knee Pain around or behind kneecap
Often worsens with downhill running Often worsens with squatting and prolonged sitting
Localized lateral knee pain Anterior knee pain
Conditions That Can Mimic IT Band Syndrome
Other causes of outer knee pain include:
-
Lateral meniscus injury
-
Lateral collateral ligament injury
-
Knee osteoarthritis
-
Referred pain from the hip
When to Seek Medical Assessment
Consider professional evaluation if:
-
Symptoms persist despite rest
-
Knee swelling is significant
-
The knee locks or gives way
-
Pain follows a traumatic injury
Typical Recovery
Most cases improve with:
-
Temporary reduction in aggravating activities
-
Hip and glute strengthening
-
Running or cycling technique adjustments
-
Mobility work
-
Load management
-
Appropriate footwear
-
Sports massage and soft-tissue treatment as supportive therapies
Key Signs of IT Band Syndrome
A classic IT band syndrome presentation is:
-
Pain on the outside of the knee
-
Symptoms that worsen during running
-
Increased pain when running downhill or descending stairs
-
Tenderness on the lateral side of the knee
-
Tightness along the outer thigh
